# MILFORD — THAILAND HEALTH ECONOMY · CAPACITY · SKILLS · ACADEMIC HUB

**Date:** 7 October 2026
**Status:** CURRENT SUPPORTING RESEARCH / BUSINESS PLAN INTEGRATION
**Control:** Latest controlled Milford information governs. This paper does not change Main by itself.

## Core proposition

Thailand’s next health-economy opportunity is not acute medicine alone. Milford’s established proposition joins the customer journey and the workforce needed to deliver it:

**MEDICAL CAPABILITY → RECOVERY → REHABILITATION → PERFORMANCE → LONGER LIFE → WELLNESS → LIVING**

underpinned by:

**PLACES + PEOPLE + SKILLS + ACADEMIC CAPABILITY**

Qualified hospitals and clinicians retain diagnosis, treatment, discharge and clinical governance. Milford owns the longer non-acute relationship around and after treatment: recovery, rehabilitation, companions, Spa/Onsen, movement, nutrition, sleep, golf, beach life, return to performance, Longer Life and, for some, residence.

## National-policy fit

Thailand’s Medical Hub architecture is organised around four linked pillars:

- **Medical Service Hub** — partner hospitals and specialists provide treatment and clinical governance.
- **Wellness Hub** — Beach Valley Spa/Onsen, Longer Life, movement, nutrition, golf, beach and destination wellness.
- **Academic Hub** — British–Thai Skills & Training Centre + University of Derby + Thai academic/vocational partners + applied research.
- **Product / innovation** — British equipment, clean technology, training rigs, water/MEP systems and live supplier training.

The July 2025 Academic Hub work addressed professional capability for medical/wellness tourism, high-value research, public/private training models and international experts. This makes the established Derby proposition materially aligned with Thailand’s national architecture rather than a stand-alone Milford education story.

## Quantified R&R capacity bridge

Working planning assumptions — not a national forecast:

| Central case | 2025 | 2030 | 2035 |
|---|---:|---:|---:|
| Medical tourists | 580,000 | 740,000 | 945,000 |
| Suitable for structured destination R&R @ 10% | 58,000 | 74,000 | 94,500 |
| Patient recovery nights @ 14 | 812,000 | 1.04m | 1.32m |
| Companion nights @ 55% × 10 nights | 319,000 | 407,000 | 520,000 |
| Total R&R-related nights | 1.13m | 1.44m | 1.84m |
| Room equivalents @ 70% occupancy | ~4,430 | ~5,650 | ~7,210 |

The room-equivalent figure is an accommodation requirement, **not** a claim that Thailand is short of exactly that number of dedicated R&R rooms.

### Milford proof requirement

**44 rooms · 68% occupancy · ~10,920 occupied nights · ~520 21-day stays · ~10 customers/week**

Against the central 2035 suitable pool, about 520 Milford stays are roughly 0.55%. The purpose of the first 44 rooms is therefore to prove referral conversion, occupancy, length of stay, price, companion demand, outcomes and operating performance before expansion.

**44 TOWER R&R ROOMS → PROVE DEMAND → CONDO / TOWER OVERFLOW → QUAD / KEEP / TOWN ONLY WHERE EVIDENCE JUSTIFIES IT**

## Physical Milford health platform

- **Beach Valley:** non-acute recovery, Spa/Onsen, movement, golf, beach, food and ordinary resort life.
- **Town Estate:** later platform for harder rehabilitation / more clinical pathways where evidence and partners justify them.
- **Sports R&R:** same 44 rooms, Spa/Onsen and clinical partner; BASEM/BDMS route and international sports relationships provide distribution.
- **Keep:** privacy-led sports/light-rehabilitation and ownership route within the current Business Plan architecture.
- **Tower / condos:** 639 existing condos; Victor’s 200-condo core programme creates longer-stay, companion, athlete and repeat-Longer-Life inventory as demand develops.

## Wellness / longevity

Thailand’s wellness economy reached **US$42.7bn in 2024**. Wellness tourism reached **US$14bn in 2024**, with latest-year growth of **36.4%**. The strategic implication is broader than medical tourism: prevention, healthspan, destination wellness, Longer Life and health-led residence form the larger funnel around the clinical base.

## Skills, Derby and the Academic Hub

Milford’s British–Thai Skills & Training Centre is enabling infrastructure, not a detached social programme.

- **Scale:** approximately **1,750 learners/year**.
- **Public benefit:** **600 free Thai places** through Victor’s 30-year covenant.
- **Covenant value:** **£13.73m**.
- **Centre capital:** approximately **£14.0m** within the established export-contract work.
- **Streams:** built environment/trades; engineering/technical; health/care/rehabilitation; hospitality/service; English/digital/employability.
- **Derby role:** curriculum and progression, quality/validation where appropriate, visiting teaching, train-the-trainer, applied research, knowledge exchange and selected study in England.
- **Thai partners:** local academic delivery, accreditation routes, vocational/practical training and employer-linked pathways.

**DERBY + THAI ACADEMIC PARTNER + THAI VOCATIONAL PARTNERS + CLINICAL PARTNERS + EMPLOYERS + MILFORD = A PRACTICAL ACADEMIC HUB PLATFORM**

## Milford as the live classroom

Milford is both consumer and training environment. The resort, R&R operation, Spa/Onsen, water and engineering systems, clean mobility, homes and construction programme become real places where people learn to build and operate the next health economy. British and Thai suppliers can train people on the systems actually installed and operated.

## Connected work recovered into this proposition

- Sports R&R / BASEM-BDMS and elite-sport referral routes.
- Gulf / Middle East medical travellers, companions, sport, hospitality and capital relationships.
- Japan / Onsen / longevity and service-rich living.
- 200-condo longer-stay route for recovery, companions, athletes and Longer Life customers.
- Keep rehabilitation and Town harder-rehab pathway.
- British supplier training and UKEF-linked export content.
- Demand Pull measurement through room nights, companion nights, treatments, F&B, repeat visits and eventual residential conversion.

## Business Plan treatment

The Business Plan must no longer present medical tourism, R&R, wellness, Longer Life and the College as adjacent propositions. They are one capacity-and-customer system.

**PLACES + PEOPLE + SKILLS + ACADEMIC CAPABILITY — ONE CONNECTED GROWTH PLATFORM**

## Superseded structure excluded

The earlier **Foundation 10% economic-interest/shareholding structure is superseded and excluded**. The current public-benefit structure is the **600 free places funded through Victor’s £13.73m 30-year covenant**. No Foundation shareholding is to be reintroduced.

## Proof discipline

**CAPITAL FOLLOWS PROOF.** The 44 rooms are the instrument that converts the national thesis into Milford evidence. Expansion follows measured referral demand, utilisation and operating performance. Proven evidence then feeds Demand Pull, What Should We Build? and the controlled model before larger R&R / Quad / Keep / Town expansion.

## Current integrated Business Plan

This work has been incorporated into:

- `Milford_The_Venture_Business_Plan_07Oct2026_ENHANCED_56pp_HEALTH_ECONOMY.pdf`
- `Milford_46pp_to_56pp_Health_Economy_Integration_Change_Record_07Oct2026.md`

The prior 46-page enhanced plan is preserved as the base; the health-economy material is added rather than replacing or compressing the existing body.
